At HeartBridge Care Services, we empower individuals with developmental disabilities to live safely, independently, and with dignity—right in the comfort of their homes and communities.
Our Supported Living Program provides personalized care and daily living support for children and adults with developmental disabilities. Whether in a private residence or a shared group setting, our caregivers focus on comfort, safety, and meaningful engagement.
We proudly serve individuals funded through Medicaid waivers, county boards of developmental disabilities, and private pay clients.
While we assist with daily living activities, our caregivers also serve as advocates and companions—helping individuals communicate their needs, pursue their goals, and fully participate in community life. Every act of care at HeartBridge builds connection, confidence, and inclusion.

Medication Management & Reminders
At HeartBridge Care Services, we are certified with MedChart, allowing our trained caregivers to safely assist with medication administration and documentation.
We ensure every client’s health routine is managed accurately and on time through:
⦁ Administering medications per physician’s orders (MedChart certified)
⦁ Timely reminders for all prescribed medications
⦁ Monitoring for potential side effects or changes in condition (non-clinical observation)
⦁ Maintaining proper medication records for accountability and family communication
